Summary

Provide person-centred housing and support to vulnerable adults, including individuals leaving prison or at risk of homelessness. The role involves assessing support needs, developing safety and support plans, assisting with housing and welfare benefits, and helping service users maintain their tenancies and develop independent living skills. Responsibilities also include managing property and tenancy issues, safeguarding, risk management, accurate record keeping, and working closely with probation, housing providers, courts, and other agencies to support positive outcomes and successful move-on into independent living.
