Summary
An opportunity to be part of a small team providing high quality specialist care to a varied group of patients. The role covers all aspects of anticoagulation and also provides a home treatment pathway for deep vein thrombosis.
Daily coordination of anticoagulation clinics including initiation, dose adjustments and counselling.
Being a expert resource for other healthcare professionals, patient and carers in both the primary and secondary care settings.
Interpretation of results and making necessary arrangements for necessary investigations and discussion of results with patients
Priority will be given to colleagues identified as at risk through the Trust’s redeployment process.
(1) Be an accountable professional acting in the best interests of people, putting them first and providing complex clinical care that is evidence-based, person-centred, safe and compassionate.
(2) Use existing knowledge and expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments.
• Run daily complex case anticoagulation clinics at NWAFT and provide daily Warfarin dose adjustment letters to patients attending anticoagulation clinics.
• Co-ordinate the management of Deep Vein Thrombosis home treatment pathway by assessing, ordering investigations and diagnosing. Organise the supply and administration of low molecular weight heparin or DOAC and commence Warfarin therapy if indicated. (Hinchingbrooke only)
• Manage and ensure the daily provision of postal Warfarin clinics including telephoning patients and seeking consultant advice where appropriate, imparting complex information acknowledging barriers to understanding.
• Assist with liaison with pre-admission clinics to prevent delayed/cancelled operations for patients on anticoagulant therapy, by offering appropriate advice.
• Communicate and liaise with the multidisciplinary team to promote effective, appropriate and quality anticoagulant care across acute trust and primary care.
(3) Act independently to plan, deliver, monitor and evaluate complex care using enhanced clinical assessments, diagnostics, and interventions
• Daily entry of Warfarin dosage onto WINPATH for all inpatients on anticoagulant therapy, to enable ward staff to be able to transcribe the Warfarin dose onto the prescription chart. (Hinchingbrooke only)
• Assist with the Trust’s adherence to NICE VTE prophylaxis guidelines, promoting, recording and investigating where necessary
(4) Act as an expert resource within their own organisation and for external agencies.
• Develop a corporate role beyond the Unit and establish appropriate networking. Host and participate in national anticoagulation meetings.
(5) Develop, deliver and evaluate education and training opportunities for others within own scope of practice.
• Provide support and education for patients and carers regarding anticoagulation therapy, in particular all new patients commenced on Warfarin and DOACS, whilst planning, advising and managing their care.
(6) Communicate effectively in challenging environments and situations with patients, their families/carers and the multi-disciplinary team.
(7) Promote and encourage innovative clinical practice to support a culture of excellence within the wider health and care team.
(8) Lead and manage unpredictable and unplanned clinical situations
(9) Participate in resource management, strategic service development, planning and service improvement.
(10) Lead, monitor, develop and appraise staff and learners.
(11) Risk/Governance - Report any potential risks, initiating appropriate action/investigation through the risk management system. Inform Directorate Manager when appropriate
