Summary
An exciting opportunity has arisen for a Clinical Pharmacist to join the Frome Medical Practice a single site Practice and Primary Care Network with just under 30000 patients who live in the attractive market town of Frome and its surrounding village. You will be working within our team of enthusiastic and highly motivated Clinical Pharmacists and Pharmacy Technicians to support patient care along with the wider team of highly skilled GPs, Nurses, Nurse Practitioners, Physiotherapist and Health Coaches. Frome Medical Practice is a recognised Learning Organisation with a strong focus on education and development our Medicines Management Team have supported a PreRegistration Pharmacy Technician to qualification, and are hoping to soon welcome Foundation Trainee Pharmacists to our team too. We are based in a fantastic purpose-built centre and are colocated with adult mental health services, district nurses, and independent pharmacy, with our local community hospital next door. We are national award winners for our work in sustainability and Green Impact initiatives, our Medicines Management Team won the national Greener PCPA Award in 2024. We have been part of the PCN Pilot Programme working with NHSE which provides exciting opportunities to shape and develop clinical pathways. As part of our Medicines Management Team, you will be responsible for optimising and ensuring the safety of our patients medication through medication reviews, chronic disease reviews, repeat prescribing, and medicines management following discharge from hospital. You will support the practice in achieving annual QOF and IIF targets, ICB prescribing incentive measures and conducting audits/quality improvement projects. You will be on a rotation as duty pharmacist, managing daily queries and urgent prescription requests, and would be expected to work one evening clinic per month. You would be part of a group of pharmacists and GPs who are keen to develop new services and enjoy project working alongside routine clinics. Recent examples include piloting an MDT for medication reviews for housebound patients, and a Cardiovascular Kidney Metabolic pharmacist chronic disease clinic. Main responsibilities Playing a key role in our Medicines Management Team to ensure medicines management optimisation and safety. This will include multidisciplinary working with other clinical teams. Undertaking structured medication reviews and chronic disease reviews with patient face to face and on the telephone, making autonomous decisions about patients medication and treatment plans. Championing the reduction of problematic polypharmacy, which is one our key outcomes as a Green Impact Practice. Reviewing and actioning any necessary changes to repeat prescribing and medicines management policies. Managing repeat prescribing process. Reconciling medicines following discharge from hospital, intermediate care and into care homes.
