Summary

Job Title: Palliative Nursing Assistant Hours: 30-36 hours per week Location: Tapping House, Hillington PE31 6BH JOB PURPOSE The Palliative Nursing Assistant is a key member of the multidisciplinary care team delivering a range of patient care duties under the supervision of the Registered Nurses and Allied Health Professionals across the Hospice, providing holistic patient care based on the physical, psychological and spiritual needs of individuals in our care. The post-holder may on occasion be required to work in other clinical areas as appropriate to meet the needs of the service as directed by the Ward Manager or nurse in charge and will participate in internal rotation to night duty and unsociable hours working to meet the needs of the service Clinical Deliver a high standard of patient care focused on individual need and promoting independence where possible. To assist in developing a caring, supportive and spiritual atmosphere in all areas of the Hospice To maintain a high standard of patient care ensuring that Hospice policies and procedures are adhered to. To give individualised patient care under the direction and supervision of the registered nurse on duty To work as a member of the ward team under the supervision of registered nurses, reporting changes in patients conditions and needs as they occur. Contribute to the multidisciplinary approach to patient care, together with other health and social care professionals and chaplaincy. Contribute to the IPU Champions programme by working with the registered nurses to promote a specific specialist topic, such as nutrition and hydration, wound care or mouth care. Provide and receive sensitive information concerning a patients medical condition. Perform care after death, maintaining the patients dignity at all times. Maintain accurate, contemporaneous patient records, in line with hospice policy. Participate in a 24-hour rota which includes days, nights, weekends and public holidays. To work in other areas of the Hospice when needed eg in the Hospice at Home team Participate in implementing care delegated by registered nurses for which training has been given and competence achieved To participate in meetings related to families and staff when requested to do so by the nurse in charge. Maintain a safe, clean and secure environment for patients, their families, visitors, volunteers and staff and report any issues identified to the nurse in charge. Education and Training Comply with mandatory training, in line with hospice policy, including restorative clinical supervision sessions a minimum of every two months. Maintain own continuing development in the field of palliative and end of life care. Participate in annual appraisal Contribute to the maintenance of the learning environment. Adopt a reflective approach to own practice with a view to continually improving Undertake further training as identified through annual appraisal or by the IPU Clinical Manager or the Clinical Educator, prior to carrying out any additional duties identified as relevant to the role Communication Communicate effectively and with dignity and respect to all patients, relatives and others, giving consideration to potential barriers to understanding Communicate effectively with colleagues, providing clear verbal, digital or written information and instructions when sharing information, delegating or handing over responsibility for care. Recognise and report any situations, behaviours or errors that could result in poor care outcomes. Contribute to a culture where all staff feel safe to raise their concerns and where speaking up about any concerns is a normal part of the routine. Help create an environment which reflects the Hospices philosophy, values and its specialist palliative care approach. Promote effective communication between members of the multi-disciplinary team and users of the service. Promote positive communications and relationships with the public. General To uphold the Hospices vision and values at all times Carry out all duties in accordance with the Hospices policies and procedures including but not limited to health & safety, code of behaviour, confidentiality and equality, diversity & inclusion. Promote awareness of the Hospice, its services and its strategy. Work collaboratively and respectfully with all staff, volunteers and supporters of the Hospice. Promote and support the safeguarding of vulnerable adults, young people and children, observing and adhering the Hospices policies on safeguarding. Demonstrate a commitment to ongoing learning and development and to participate in any training relevant to the role. This post is subject to the Rehabilitation of Offenders Act 1974 Exemptions Order 1975, and such will be necessary for a submission for disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions. Information Security, Confidentiality, and Records Management: The role requires adherence to organisational policies concerning information security, data privacy, and confidentiality. The employee is responsible for safeguarding sensitive information, ensuring secure handling and storage of records, and maintaining compliance with applicable data protection regulations. This includes actively participating in Information Governance training, reporting potential security incidents, and managing records in accordance with Tapping Houses Retention Archiving and Destruction of All Hospice Records Policy.
