Summary

We are looking for a registered mental health practitioner to join the new Crisis Access Hub as a Band 6 Clinical Screening Practitioner. This is a key role in the Crisis Pathway Redesign Programme, supporting timely and consistent access to the right crisis support across Kingston, Merton, Richmond, Sutton and Wandsworth.

The role is focused on telephone and electronic referral screening, prioritisation, signposting and onward routing. It does not include face-to-face assessment, home visits or treatment. You will work within agreed clinical frameworks, using judgement to identify risk, urgency and the most appropriate pathway, with escalation to Band 7 where required.

The post holder will screen referrals into the Crisis Access Hub using agreed protocols, clinical records and information from referrers to support safe pathway decisions. You will contribute to the 1-hour screening target by managing your own referral workload, documenting decisions clearly and escalating delays, complexity or uncertainty to the Band 7 Senior Clinical Practitioner. You will identify risk, safeguarding concerns and immediate safety issues, apply the trusted assessor principle where appropriate, and route referrals to the right onward service. The role requires sound clinical judgement, clear communication, accurate documentation and the ability to work calmly in a high-demand crisis access environment.
