Summary

We are looking for a qualified Mental Health and Wellbeing Practitioner with a passion for working with older people, to join our friendly Older Adult Community Mental Health Team in Bolton. You will be working closely as part of our Therapies Hub, which consists of two Clinical Psychologists, one CERN Practitioner, and a Mental Health and Wellbeing Practitioner. You will also play a key role in the wider multi-disciplinary team, working closely with Psychiatrists, Clinical Psychologists, a CERN Practitioner, Nurses, Occupational Therapists, a Physiotherapist, Carer Support Workers, Physical Health Professionals, and Support Workers.

The post holder will work under close case management supervision from a suitably qualified professional to provide high quality collaborative care planning as a member of the multi-disciplinary team. The post holder will work under close clinical skills supervision from an HCPC registered clinical psychologist to provide specified psychologically informed interventions, with a level of autonomy matched to level of demonstrated competence.

· Carryoutrelevantriskassessmentsandriskmanagementwiththemulti-disciplinary team.

· Engagecalmlyandwithsensitivityandempathytosupportserviceusers in highly distressing or emotional circumstances.

· Engagewithandgatherinformationfromserviceusers,relativesand patient records to develop a formulation.

· Toworkincollaborationwiththeindividualserviceuserandtheirfamily and carers as appropriate to develop and contribute to care plans that are focused on strengths and are outcome based.

Toattendmulti-disciplinaryreviewsforacaseloadofserviceusersin order to support and monitor progress during the course of multi-disciplinary interventions.

· To deliver specified wellbeing-focused psychologically informed interventions, in line with best available evidence, under close supervisionfromaclinicalpsychologist.Thesewill include but are not limited to:

o Improvingsleep

o Recognisingandmanagingemotions

o Buildingconfidence

o Relapse prevention

· Toberesponsivetoserviceusers’needsandchoices;andupholdtheirrighttobetreatedwithdignityandrespect.

· Toincludecarersandfamiliesinlinewiththeserviceuser’swishes.

· To liaise with other health and care providers, including third-sector agenciesandprimarycare,toensurecontinuityofcareforserviceusers.

· To communicate information concerning the assessment, formulation andcareplansofserviceusersinaskilledandsensitivemannerinorder to promote effective multi-disciplinary working and therapeutic outcomes for clients.

· Todevelopcollaborativeplansforrelapseprevention.

Todealwithendingsappropriatelyandsafelywithserviceusers,families and carers.

· Toformprofessionalrelationshipswithserviceusersandcommunicate with them in a way that respects their views, autonomy and culture.

· Tomonitorandrecordprogression,highlightconcernsandreportany

perceivedchangesinserviceusers’wellbeingtotheteam.

· To demonstrate effective communication skills in discussing highly sensitiveandcomplexinformationappropriatelywithpeoplewhose understanding may be temporarily or permanently impaired.

· To provide relevant information, written and oral, for records and reviewsrelatingtoserviceusers’progressundersupervisionfroma suitably qualified member of the team.

· Providehighlyeffectivewrittencommunicationsasrequiredforservice users and other health and care professionals.

· Reportregularlyandindetailtotheclinicalskillssupervisorandcase management supervisor.
