The Salvation Army is looking for a compassionate and committed Support Worker to join our Homelessness Services team at our Lifehouse - Southward Young Peoples Service.This is an opportunity to play a key role in delivering high-quality, person-centred support to people experiencing homelessness or vulnerability. Working as part of a dedicated team, you will support clients to identify their goals, access the right services and develop the skills and resilience needed to move towards more sustainable lifestyles.As a Support Worker, you will develop and deliver individual support plans that promote client choice, independence and control. You will complete assessments, review progress, coordinate support activities and work closely with colleagues and external partners to help clients achieve positive outcomes.You will work in a trauma-sensitive and psychologically informed way, ensuring support is strengths-based, respectful and responsive to each person’s circumstances.Key responsibilities include:Developing, reviewing and updating individual support plans with clients.Completing client assessments, including risk assessments, within agreed timeframes.Helping clients access benefits, housing options, health services and other external support where needed.Working with colleagues to design and deliver in-house support activities that contribute to successful outcomes.Maintaining accurate records using The Salvation Army’s client management system.Supporting strong communication across the team so clients receive joined-up, consistent support.Ensuring practice is safe, professional and aligned with relevant policies, procedures and legislation.To be successful in this role you do not need to have followed one particular career path to succeed in this role.
