Summary
The Care Co-ordinator plays an important role in supporting the delivery of high-quality, patient-centred care. Working as part of the multidisciplinary team, the post holder will coordinate care for patients with complex, long-term or multiple health needs, ensuring they receive timely, appropriate, and well-coordinated support. The role involves working closely with GPs, nurses, practice staff, community services, and external organisations to improve communication, reduce barriers to care, and enhance the patient experience. The Care Co-ordinator will provide administrative and coordination support while helping patients navigate health and care services.
