Job Title: Care Coordinator Base: Winshill Medical Centre Responsible to: Practice Manager / GP Partners Hours: Full time, 37.5 hours per week (MondayFriday, 09:0017:00) Occasional out-of-hours/weekend work may be required depending on service needs Salary: Depending on experience (DOE) Job Summary Job Title: Care Coordinator Base: Winshill Medical Centre Responsible to: Practice Manager / GP Partners Hours: Full time, 37.5 hours per week (MondayFriday, 09:0017:00) Occasional out-of-hours/weekend work may be required depending on service needs Salary: Depending on experience (DOE) Job Summary The Care Coordinator will provide a combination of care coordination and practical support for patients across 11 nursing, residential, and learning disability (LD) homes, as well as a dedicated respite care facility. Working closely with the multidisciplinary team, you will help ensure the delivery of high-quality, person-centred care and support the smooth day-to-day running of services. The role involves regular travel between care homes to support patient reviews, care planning, proactive care management, and communication between healthcare professionals, care home staff, patients, and families. Main Duties and Responsibilities Coordinate and support person-centred care across 11 nursing, residential, and learning disability (LD) homes and a respite care facility. Conduct monthly visits to care homes to review resident needs, monitor care plans, and support proactive care management. Undertake weekly visits to the respite care home to support resident reviews, care coordination, and communication with staff. Act as a key liaison between the practice, care homes, residents, families, and healthcare providers to ensure continuity of care. Support care planning, patient reviews, and follow-up actions as directed by the multidisciplinary team. Coordinate referrals, appointments, investigations, and healthcare interventions to ensure timely care delivery. Maintain accurate and up-to-date patient records in line with practice policies and regulatory requirements. Monitor residents requiring enhanced support and escalate concerns appropriately to clinical staff. Assist with audits, quality improvement initiatives, service development, and data collection. Provide administrative and operational support to ensure effective care home service delivery. Build and maintain positive working relationships with care home staff, residents, families, and external agencies. Travel independently between care homes and practice locations as required. Person Specification (Essential) Excellent communication and interpersonal skills Strong organisational and time management skills Ability to work independently across multiple sites Experience in a healthcare, social care, or administrative coordination role Desirable Understanding of clinical terminology and care pathways (beneficial but not essential) Experience working with care homes, including frail elderly or LD populations Knowledge of primary care systems (EMIS Web, AccuRx, Docman 10)
