Summary

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

We are looking for an experienced Band 3 Healthcare Assistant to work as part of an established community heart failure service providing support and health care to patients and carers within their home environment and or clinic setting, under the direction of the Community Heart Failure Nurse Specialist. This includes assisting in the ongoing assessment, provision of care and reporting changes in patient status to the nurses.

You will be expected to become proficient and competent in manual blood pressure and heart rate monitoring, assessment of fluid status and venepuncture. Undertaking competency assessments specific to the area of clinical practice. This role involves promoting optimum independence through the provision of client education, and individualised client centred care to a delegated caseload.

This is an exciting opportunity for an individual looking for ongoing training and development with scope to develop the role into a nursing associate. Supervision and multi-disciplinary working are our priorities to ensure the best care for patients.As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).

The role will include, but is not limited to:

• Supporting the Heart Failure Team to deliver against key performance indicators to maintain the quality of the service and to input data for clinical outcome measures.

• Assisting the Community Heart Failure Specialist Nurses in the care planning, assessment, monitoring and review process of patients referred to the Community Heart Failure Service.

• Independently visiting patients in their own home to carry out a basic heart failure assessment and undertake simple clinical tasks such as weight, blood pressure, pulse and oxygen saturation monitoring, venepuncture sample collection, skin care inspection and prevention of pressure sores as delegated under the guidance of the Community Heart Failure Nurse.

• Supporting the management of patients out of and into Heart Failure Virtual Hospital

• The role involves working closely with GP’s, other members of the integrated community teams, and with social care partners. As such you will be required to have excellent communication skills and be confident to work using your own initiative as well as being an excellent team player.
