Summary
CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are seeking a Band 4 TB Support Worker to join our Tuberculosis (TB) Service. This role is central to supporting individuals undergoing TB treatment and contributes to improved treatment adherence, health outcomes, and overall wellbeing.
You will work in close partnership with TB Specialist Nurses and the wider multidisciplinary team to deliver high-quality, patient-centred care in both community and clinic settings.
As part of the TB Service, you will support adults, children, and community groups, engaging with a diverse population to raise awareness of TB, reduce new cases, promote successful treatment outcomes, and ensure a positive patient experience.
• Support individuals with tuberculosis (TB) to follow their treatment plans, including delivering Directly Observed Therapy (DOT) where required
• Provide education and advice to patients, families, and carers about TB, treatment and infection control
• Assist with contact tracing and the follow-up of patients in community settings
• Support vulnerable and hard-to-reach populations to access TB services and appropriate care
• Monitor patient progress and promptly report any concerns or changes to the clinical team
• Maintain accurate, up to date, and confidential patient records in line with policy
• Support TB notifications under the supervision of the TB Clinical Nurse Specialist (CNS)
• Contribute to multidisciplinary team (MDT) meetings and service audits as required
• Assist with monitoring treatment adherence for patients receiving TB therapy
• Undertake home visits alongside the TB Clinical Nurse Specialist (CNS)
• Carry out phlebotomy procedures in accordance with training, competency, and guidance
• Work collaboratively with healthcare professionals and external agencies to ensure coordinated, patient centred care
