Summary
As a Care Coordinator within West Northumberland Primary Care Network, you will play a key role in supporting patients to access appropriate health, care and community services, ensuring they receive coordinated and personalised support tailored to their individual needs. Working closely with GP practices and members of the multidisciplinary team, you will proactively identify and support patients who may benefit from care coordination. You will act as a central point of contact, helping patients and carers navigate services, manage appointments, access relevant support, and develop personalised care and support plans. The role involves building strong relationships with patients, healthcare professionals and partner organisations to promote joined-up care and improve patient outcomes. You will contribute to the coordination of multidisciplinary team meetings, support the management of patient caseloads, and ensure that care plans and relevant information are effectively communicated and recorded. You will also support the delivery of key Primary Care Network priorities, including cancer care coordination and, where required, work associated with Enhanced Health in Care Homes. This may include supporting patients through screening and referral pathways, assisting with follow-up activity, and helping individuals and their families understand and access the support available to them. The successful candidate will demonstrate excellent communication and organisational skills, be confident working both independently and as part of a team, and have a passion for delivering high-quality, person-centred care.
