Summary

JOB DESCRIPTION Job Title Healthcare Support Worker Hours24 hours per week worked Tuesday to Friday 8am 4.30pm (30 minutes unpaid lunch break) Responsible To Practice Manager and GP Partners Job Summary The Healthcare Support Worker will support the clinical team in delivering high-quality, patient-centred care within General Practice. The postholder will undertake a range of delegated clinical activities, support preventative healthcare and screening programmes, contribute to the management of patients with stable long-term conditions, and assist clinicians in the delivery of safe and effective services. The role is part funded through the Primary Care Network Additional Roles Reimbursement Scheme (ARRS) and part funded by White Medical Group. Main Duties and Responsibilities Clinical Duties Undertake venepuncture (phlebotomy) Measure and record blood pressure, pulse, height, weight and BMI. Perform basic clinical observations in accordance with practice protocols. Collect and process specimens as required. Assist with infection prevention and control procedures. Chaperone patients when required. Assist clinicians with the administration of injectable medications, including Vitamin B12 injections, in accordance with practice protocols, training, competence, and patient specific directions where applicable. Health Promotion and Screening Support the delivery of health promotion initiatives. Undertake lifestyle assessments covering smoking, alcohol consumption, diet, exercise and general wellbeing. Provide brief lifestyle advice and signposting to relevant services. Support national and local screening programmes, including cancer screening initiatives. Encourage patient engagement with preventative healthcare services. Long-Term Condition Support Assist with annual reviews for patients with stable long-term conditions in accordance with agreed protocols. Support recall programmes and patient monitoring processes. Undertake data collection and recording for chronic disease management reviews. Escalate concerns or abnormal findings to an appropriate clinician. Administration Accurately record consultations and clinical information within the clinical system. Ensure patient records are maintained to a high standard. Assist with recall systems and population health initiatives. Participate in audits and quality improvement activities as required. Professional Responsibilities Maintain confidentiality at all times. Work within practice policies and procedures. Attend mandatory training and participate in continuing professional development. Maintain professional standards and competencies. Demonstrate a commitment to equality, diversity, and patient-centred care. Carry out any other reasonable duties commensurate with the nature, level and responsibility of the role.
