Summary

Main Duties and Responsibilities: Clinical Responsibilities: Provide general medical services to registered patients during scheduled clinical sessions. Undertake face-to-face, telephone and video consultations as appropriate. Assess, diagnose, treat and manage acute and chronic medical conditions. Prescribe medications in accordance with NHS and local prescribing guidelines. Refer patients to secondary care and other healthcare professionals when appropriate. Review pathology results, correspondence and clinical documents. Undertake home visits where required. Promote preventative healthcare and health education. Maintain accurate and contemporaneous patient records using the clinical system. Patient Care: Deliver safe, effective and compassionate healthcare. Ensure continuity of care whenever possible. Support patients in making informed decisions regarding their care and treatment. Safeguard vulnerable adults and children in accordance with local and national policies. Quality and Governance: Participate in clinical governance activities. Contribute to Quality and Outcomes Framework (QOF) achievement. Support quality improvement and audit projects. Report significant events and participate in learning activities. Maintain compliance with GMC standards and practice policies. Multidisciplinary Team Working: Work collaboratively with GPs, nurses, pharmacists, physician associates, healthcare assistants and administrative staff. Attend practice and multidisciplinary team meetings as required. Contribute to the development of integrated patient care pathways. Administrative Responsibilities: Process clinical correspondence, pathology results and prescriptions within agreed timescales. Complete medical reports and statutory documentation when required. Maintain accurate clinical coding and record keeping. Teaching and Development Participate in continuing professional development (CPD) activities. Maintain annual appraisal and GMC revalidation requirements. General Responsibilities: Comply with all practice policies and procedures. Maintain confidentiality at all times in accordance with GDPR and NHS guidance. Follow health and safety requirements. Promote equality, diversity and inclusion. Undertake any other reasonable duties commensurate with the post.
