Summary
PCN Social Prescriber's provide personalised support to individuals, their families, and carers to take control of their well-being, live independently, and improve their health outcomes. The role will focus on strengthening personal resilience and reducing health inequalities by addressing wider determinants of health such as debt, poor housing, physical inactivity, and low understanding of health conditions. The Social Worker will take a holistic approach, working closely with individuals to co-produce personal action plans, reconnect them with community resources, and encourage them to achieve their health and well-being goals.
