Summary
We are seeking an organised, proactive, and patient-centred Discharge Care Co-Ordinatorto join our team at Frimley Health NHS Foundation Trust. This pivotal role supports the safe, timely, and effective discharge of patients from hospital settings. Working closely with the multi-disciplinary team (MDT), the post-holder will co-ordinate discharge planning processes, support communication with families and community services, and ensure discharge arrangements are in place and progressing efficiently.
This is a ward-based role, typically assigned to a named ward or group of wards. However, the post-holder will be required to provide cross-site support and flexible cover at times of operational demand, acrossWexham Park, Heatherwood, and Frimley Park Hospitals.

You will act as a key point of contact for patients, relatives, carers and professionals on your allocated ward, providing advice, coordination and support throughout the discharge process.
You will maintain momentum throughout the discharge planning process, working in partnership with doctors, nurses, occupational therapists, physiotherapists and other members of the Hospital Assessment and Discharge Team. You will contribute to MDT discussions, identify and address potential barriers to discharge, and constructively challenge decisions where appropriate.
You will also:
• Coordinate and contribute to comprehensive discharge planning from admission through to discharge.
• Liaise effectively with patients, families, carers, social services and external agencies to ensure appropriate support is in place.
• Identify potential delays or barriers to discharge and take proactive action to resolve these.
• Promote patient choice, independence, dignity and involvement in decisions about discharge and ongoing care.
• Support effective communication and information sharing across the multidisciplinary team and partner organisations.
• Ensure discharge arrangements are safe, appropriate and aligned with the patient’s assessed needs.
• Escalate concerns, risks or delays appropriately and contribute to solutions that support effective patient flow.
• Develop positive working relationships with colleagues and partner agencies to support integrated care and timely discharge.
