Summary
Key Responsibilities 1. Strategic Leadership & PCN Business Planning Lead the development, delivery, and annual refresh of a PCN strategy and business plan, ensuring alignment with Federation objectives and practice priorities. Translate DES requirements into clear operational plans with measurable outcomes. Ensure centralisation of DES related workstreams wherever appropriate, reducing duplication and variation across PCNs. Provide structured leadership support to Clinical Directors, enabling them to focus on clinical leadership while the postholder oversees operational delivery. Act as a strategic link between practices, PCNs, the Federation Board, and system partners (ICB, INT, Local Authority). Work collaboratively with Federation leadership to align PCN business planning with Federation strategy, ensuring shared priorities are reflected in operational delivery and transformation programmes Incorporate SNP and MNP neighbourhood development work, ensuring PCN plans complement neighbourhood operating models and INT development. 2. Programme & Transformation Management Lead planning, implementation, and evaluation of transformation programmes including personalised care, digital and data initiatives, workforce development, and neighbourhood level projects. Develop programme documentation, risk logs, delivery plans, and progress reports. Ensure DES specifications are met, evidenced, and reported accurately. Identify opportunities for centralisation of workstreams (e.g., care coordination, data reporting, patient engagement, workforce planning). Oversee development and implementation of new services supporting population health management and improved patient outcomes. Work collaboratively with Federation, INT partners, and ICB colleagues to support neighbourhood level transformation and proactive care models. 3. Workforce Leadership & Line Management Provide day to day leadership, supervision, for ARRS staff, ensuring clarity of role, workload management, and high quality performance. Support ARRS staff to work consistently across practices, embedding standardised processes and shared operating models. Work with the Federation HR Manager to ensure robust workforce planning, recruitment, supervision, and development. Promote a culture of collaboration, continuous improvement, and professional growth within the PCN workforce. 4. Practice Engagement & Relationship Management Build strong, trusting relationships with member practices, ensuring transparent communication and shared ownership of PCN priorities. Actively engage practices in shaping PCN strategy, DES delivery, and transformation programmes. Facilitate regular practice engagement forums, feedback loops, and collaborative decision making structures. Ensure practices feel supported, listened to, and central to the PCNs direction and delivery. Ensure consistent communication and joint working between practices, PCNs, and the Federation to support cohesive planning and delivery. 5. Partnership Working & INT Integration Act as the operational lead for PCN involvement in the Integrated Neighbourhood Team. Strengthen relationships with community providers, mental health services, social care, voluntary sector organisations, and wider system partners. Ensure PCN priorities are represented within INT planning and that neighbourhood level initiatives are embedded across practices. Work collaboratively with Federation and ICB colleagues to support neighbourhood development, including SNP and MNP neighbourhood priorities, MDT development, and proactive care models. Support the maturation of INTs, ensuring alignment with national policy, local neighbourhood plans, and the 10 Year Health Plan. 6. Data, Reporting & Performance Oversee data collection, analysis, and reporting for DES requirements, transformation programmes, and performance monitoring. Work with Federation analysts to produce actionable insights for Clinical Directors and practices. Ensure compliance with national reporting requirements and support practices to understand performance trends and improvement opportunities. Support neighbourhood level population health management, risk stratification, and proactive care planning. 7. Governance & Quality Improvement Ensure robust governance arrangements for PCN DES delivery, including risk management, financial oversight, and quality assurance. Support practices with QI methodologies, shared learning, and collaborative improvement cycles. Work with the Federation governance team to ensure policies, procedures, and reporting structures are adhered to. Ensure governance alignment across PCNs, Federation programmes, neighbourhood structures, and INT working. This job description will be reviewed regularly and amended in agreement with the post-holder and the Primary Care Network Clinical Director and or Federation Managing Director.
