Summary
Clinical Care & Professional PracticeProvide holistic mental and physical health support in our community clinic, including venepuncture, physical observations, medication ordering and preparing patients for procedures. Monitor changes in patient wellbeing, escalate concerns appropriately, and promote health through advice and ongoing support. Act as a patient advocate, ensuring dignity, equality and inclusion. Teamwork & CommunicationWork collaboratively within the multidisciplinary team, contributing to smooth clinic operations and safe patient care. Communicate clearly with colleagues, patients and families, participate in handovers and MDT meetings, and handle verbal concerns sensitively. Organisation, Safety & GovernanceManage and prioritise your workload effectively. Adhere to Trust policies on medicines management, infection prevention, safeguarding and confidentiality. Support risk assessments, incident reporting and quality improvement, while ensuring equipment is used and maintained safely. Learning, Development & SupervisionEngage in supervision, mandatory training and ongoing development. Support students and junior staff where appropriate and work within your professional limits in line with the NMC Code.
