Summary
To work collaboratively within the general practice team to meet the needs of patients. Provide evidence-based care, working within guidelines and policies. Recognise and work within his/her own competence and within a professional code of conduct as regulated by the NMC. Clinical Practice Phlebotomy and other sample taking such as wound or PV swabs. Blood pressure measurement; acting on and escalating the result according to policy and competence. Urinalysis; acting on and escalating the result according to policy and competence. ECG and ambulatory blood pressure recording. Assessment, evaluation and treatment of post operative, acute and chronic wounds. Administration of injectable medication. Health promotion: advice and signposting to services such as smoking cessation and Health coaches. Work with other health care professionals to diagnose, monitor, manage and treat long term conditions. Recognise the need for, and assist in the provision of, urgent and emergency care. (Following appropriate training and competency assessment): Implement and participate in vaccination programmes for adults and children. To include taking part in the annual flu campaign (this will involve potentially working maximum of 2 Saturday clinics in each year). Cervical cytology sample taking. Assessment for and administration of travel vaccinations. Care for patients presenting with aural conditions, to include ear syringing. Providing contraception advice and review. Using clinical decision-making software appropriately to monitor patients on warfarin. Performing ABPI assessment; using the result in planning of wound care and/or escalating the result according to policy and competence. Learning and development Complete mandatory training as required. Take responsibility for own learning and development including revalidation requirements and maintenance on the NMC register. Supervise (where appropriately qualified) and act as role model to student nurses. Participate in nurse meetings including disseminating learning and information to share good practice. Participate as required in processes to improve quality of patient care: audit, quality improvement, clinical supervision, colleague feedback, significant event analysis, and appraisal. Participate as required in management and review of patient concerns and complaints, recognising that effective and empathetic listening and communication can often avoid the escalation of a concern. Delivering a quality service Have respect for patients privacy and dignity, and apply the principles of confidentiality appropriately. Promote and adhere to infection control policies and measures including daily cleaning schedule, appropriate use of PPE, and maintenance of the cold chain. Produce accurate, contemporaneous and complete records. Prioritise, organise and manage own workload in a manner that maintains and promotes quality Promote equality and diversity, supporting patients who need assistance in exercising their right to access health care. Recognise and refer patients presenting with mental health needs. Recognise, and escalate appropriately according to policy, concerns around child and adult safeguarding.
