Summary

Provide specialist nursing care to heart failure patients in both inpatient and outpatient settings. Undertake clinical assessments and develop personalised care plans. Manage a clinical caseload of patients with support from the heart failure nursing lead and consultant colleagues and the wider multi-disciplinary team. Support patients in self-management and improving quality of life. Contribute to heart failure service development and quality improvement initiatives. Deliver education and training to other healthcare staff to enhance heart failure care across the Trust. Work closely with other member of the team cardiologists, GPs, pharmacists, community heart failure nursing teams and wider multidisciplinary teams. Escalating concerns to appropriate medical teams, acting as patients' advocate and operate in the patients best interest. To provide an effective line of communication for patients and carers including telephone support. Referral of patients to appropriate health and social care partners.
