Summary
About the role:
This post plays a key role in supporting the delivery of integrated commissioning and service transformation projects between Powys County Council (PCC) and Powys Teaching Health Board (PTHB).
The role focuses on enabling partnership-led initiatives that improve system flow, reduce duplication, and support timely access to care. This includes the development and implementation of shared pathways, governance arrangements, standard operating procedures (SOPs), referral processes and performance reporting, rather than the direct operational delivery of care packages.
The post holder will provide project support, analytical capacity and coordination across a number of active joint workstreams, particularly those relating to integrated brokerage arrangements, Trusted Assessor models and future integrated approaches to Direct Payments. The role is central to ensuring that joint commissioning arrangements are underpinned by clear processes, strong data and robust reporting.
About you:
You will have experience of working in partnership environments, ideally across health and social care, with a strong understanding of how commissioning, funding responsibilities and operational systems interact.
You will be confident supporting complex projects involving multiple stakeholders and
organisations, and comfortable developing documentation, workflows, guidance and reporting frameworks to support service delivery at scale.
You will be analytical, organised, and skilled at translating policy and strategic intent into practical processes that can be consistently applied across organisations.
What you will do:
Support the design and delivery of joint commissioning and integration projects between PCC and PTHB
Develop and document pathways, referral processes, SOPs and supporting materials for partnership initiatives
Coordinate activity across internal teams and external partners to ensure initiatives are implemented consistently
Produce performance data, dashboards and reports to monitor outcomes, activity and compliance
Provide project support across the full commissioning cycle, excluding direct case brokerage
This position has a requirement for an Enhanced DBS Check
