Summary
The Support Time Recovery (STR) Worker will work across the Crisis Pathway, providing both telephone-based crisis triage and community-based recovery-focused interventions to individuals experiencing mental health crises.
The post holder will support service users and carers by delivering person-centred, recovery-focused interventions that promote independence, social inclusion, safety and wellbeing. The role involves providing telephone triage, crisis support, risk assessment, signposting, practical support and physical health monitoring under the supervision of qualified mental health professionals.
The post holder will work collaboratively across Crisis Resolution and Home Treatment Teams (CRHTT), Centralised Crisis Line services, community mental health teams, inpatient services and external agencies to ensure service users receive timely and appropriate care.
The role requires sound judgement, effective communication skills and the ability to work independently while recognising the need for supervision and escalation of complex or high-risk situations.
• Receive and triage calls from service users, carers, professionals and partner agencies presenting with mental health concerns or crises.
• Undertake initial mental health and risk screening assessments.
• Identify immediate risks including suicide, self-harm, self-neglect, safeguarding concerns, violence or deterioration in mental state.
• Escalate concerns appropriately to qualified mental health practitioners.
• Develop and contribute to crisis safety plans under clinical supervision.
• Determine appropriate pathways of care and signpost callers to relevant services.
• Support delivery of crisis interventions as an alternative to hospital admission.
• Recognise when physical health needs take precedence and seek support regarding escalation to NHS 111 or emergency services.

• Assist qualified practitioners in conducting crisis assessments and gatekeeping processes.
• Participate in multidisciplinary reviews and care planning.
• Monitor and report changes in mental state, behaviour and risk.
• Identify early warning signs of relapse and deterioration.
• Undertake physical health monitoring including:• Blood pressure
• Pulse
• Temperature
• Respiratory rate
• Oxygen saturation
• Weight and BMI
• Report abnormalities to registered clinicians promptly.
• Deliver brief interventions and health promotion advice.
• Contribute to safeguarding assessments and interventions.
