Summary
Roles and Responsibilities
Undertake strengths-based social work assessments for older people and adults requiring support, ensuring their needs, wishes, and outcomes are at the centre of care planning.
Work collaboratively with individuals, their families, carers, and partner agencies to promote independence, wellbeing, and positive outcomes.
Support the safe and timely discharge of patients from hospital by assessing ongoing care needs and coordinating appropriate community support services.
Develop, implement, and regularly review person-centred care and support plans in line with relevant legislation, policies, and best practice.
Carry out safeguarding enquiries where required, identifying and managing risks to vulnerable adults while promoting their rights and wellbeing.
Work as part of a multidisciplinary team, liaising with healthcare professionals, community services, and voluntary organisations to deliver coordinated care.
Maintain accurate, timely, and confidential case records, assessments, reports, and care plans using electronic case management systems.
Provide information, guidance, and advocacy to individuals, families, and carers, helping them understand available services and make informed decisions.
Manage a varied caseload effectively, prioritising work to meet statutory timescales and organisational standards.
Ensure all practice complies with relevant legislation, Social Care Wales professional standards, safeguarding procedures, and organisational policies.
