Summary
JOB DESCRIPTION

JOB PURPOSE
To provide a range of personal, social, emotional and practical care tasks as required for the mental and physical well being of service users living in their own homes within the community.
a)To ensure compliance with company policies and all statutory and regulatory requirements. Code of conduct under skills for care.
b)To work towards an assessed and planned care or support and maintenance packages thus enabling service users and their carers to exercise control over own lives by maximising their choice and independence.
c)To provide a culturally and ethnically sensitive service. At all times to work within equal opportunities policy and procedure.
d)To ensure users retain their Dignity and Individuality through a flexible and appropriate level of service.
2. DUTIES & RESPONSIBILITIES
Will incorporate the proposals of all appropriate legislation and will be carried out following an assessment and instruction from the Registered/Deputy Manager.
a)Have individual responsibility under the supervision of Registered Manager/Deputy for the personal needs of service users.
b)Support the development of person centred planning, ensuring each individual’s plan has meaningful and achievable goals.
c)Work in partnership with care managers and other professionals to maximise quality of life for individuals
d)Providing personal care in a sensitive manner for all service user groups and their carers living in their homes within the community (this may include assisting service user to get up and go to bed, total body washing, in line with their person-centred care plan, assisting with toileting, dressing and feeding) and/or assisting with parenting skills and providing carers support.
e)Work in line with the company’s Adult Protection and Prevention of Abuse Policy and Whistle Blowing Policy to ensure that people are kept safe at all times
f)Support people to take an active role in the community by accessing local facilities and wider community resources in the spirit of social inclusion.
Social duties include shopping; talking with service users, helping to maintain contact with family, friends and the local community, escort duties which may include e.g:
1.Supporting service users, e.g. to the shops/bank.
2.Supporting service users to e.g. G.P. and health needs in accordance to each individual person ‘s health action plan.
3.Work as a keyworker/ co keyworker to coordinate and develop the support to the individual including Health Action Plans and Person-Centred Plans.
g)Underrtake essential domestic duties to assist service users to care for their homes (e.g. cleaning, cooking, laundry), whilst encouraging service users to retain maximum independence by creating a supportive homely atmosphere.
h)Referrals to the Registered Manager for equipment or services needed by service users (e.g. meals-on-wheels, incontinence supplies).
i)Assisting service users where instructed by the Manager in the management of their income, paying rent and other household bills and keeping a strict account for each transaction.
j)To attend Training Courses as required to develop skills and knowledge to improve the quality of service provision.
k)Maintain professional knowledge and competence.
l)Attend mandatory training days/courses, on or off site, as and when required.
m)Carry out any other duties and responsibilities within the scope and spirit of the job purpose and grade as may be required.
n)The role is essential for using a device and CRM system.
3. Special Conditions
In order to facilitate meeting the needs of our service users, which operate 365 days per year it is an essential requirement of the job that post- holders will be required to work flexible and unsocial hours. This will include some evening, weekends, nights and Bank holidays. Job Specification

5. Development
The employee has a responsibility to participate in the regular appraisal with their line manager and identify performance standards for the post. As part of the appraisal process there is a joint staff responsibility to identify learning and development needs to meet the performance standards required of the post holder
6. Health & safety at Work
Every employee is expected to take care of themselves and other people who may be affected by their actions or inactions. As such it is the responsibility of employees to act safely at all times and where appropriate in accordance with their relevant professional body’s standards. Employees are expected to adhere to Health & Safety legislation including their obligations as defined by the Working Time Regulations.
Travelling
The post will require travelling between base, service user’ homes, other Desired Care 4 U premises and any alternate venues to provide services
Confidentiality and Integrity
Within Desired Care 4 U there is often a need to use or be party to confidential information. The unauthorised use or disclosure of any user or other personal information, whether in verbal, written or electrical format, or through negligence is regarded as misconduct that can become subject to Desired Care 4 U disciplinary procedure. It could also result in prosecution or civil action arising from various legislative acts including the data protection act.
Staff are expected to act with integrity both inside and outside of the workplace. Acts that could cause disrepute to the PCT or call into question a person’s suitability to continue in their role may have to be regarded as issues of gross misconduct.
