Summary
The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. Working as part of a multidisciplinary team, the post holder will contribute to assessment, care planning, risk management and the delivery of person-centred, recovery-focused care. The post holder will work collaboratively with service users, families and carers to develop strengths-based care plans, coordinate interventions and support shared decision-making. They will provide evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, problem-solving, emotional wellbeing support, sleep management, confidence building, medication support and relapse prevention planning. Key responsibilities include carrying out risk assessments, monitoring service users progress, maintaining accurate clinical records, contributing to multidisciplinary discussions and liaising effectively with healthcare professionals, social care and third-sector organisations to ensure continuity of care. The successful candidate will demonstrate compassion, empathy and excellent communication skills when working with individuals experiencing distress or complex mental health needs. They will actively participate in clinical and case management supervision, quality improvement activities and ongoing professional development, maintaining high standards of clinical practice and professional conduct at all times.
