Summary

We are seeking a motivated and compassionateMental Health and Wellbeing Practitioner (MHWP)to join our Older Adults Community Mental Health Team. This is an exciting opportunity to support older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty, within a multidisciplinary community setting.

The post holder will work collaboratively with service users, families, carers and colleagues to coordinate person-centred care, promote recovery, and support positive wellbeing outcomes. Under the supervision of experienced mental health professionals and a Clinical Psychologist, the successful candidate will deliver evidence-based, psychologically informed interventions aligned to cognitive behavioural principles, including behavioural activation, problem-solving, emotional wellbeing support, sleep interventions and relapse prevention planning.

This role offers the opportunity to develop specialist skills in older adult mental health while contributing to a compassionate, strengths-based service that values dignity, inclusion, collaborative care planning and evidence-based practice.

The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. The post holder will work collaboratively with service users, families, carers and the multidisciplinary team to develop and deliver person-centred, strengths-based care plans that promote recovery, wellbeing and independence.

Key responsibilities include undertaking risk assessments and contributing to risk management plans; gathering and analysing information to inform assessment, formulation and care planning; acting as a named worker for allocated service users; and coordinating care to promote continuity and positive outcomes.

The post holder will deliver evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, graded exposure, problem-solving, emotional wellbeing support, sleep interventions, confidence building, medication support and relapse prevention.

Working as part of a multidisciplinary team, the post holder will contribute to assessment, care planning, risk management and the delivery of person-centred, recovery-focused care.

The post holder will work collaboratively with service users, families and carers to develop strengths-based care plans, coordinate interventions and support shared decision-making. They will provide evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, problem-solving, emotional wellbeing support, sleep management, confidence building, medication support and relapse prevention planning.

Key responsibilities include carrying out risk assessments, monitoring service users’ progress, maintaining accurate clinical records, contributing to multidisciplinary discussions and liaising effectively with healthcare professionals, social care and third-sector organisations to ensure continuity of care.

The successful candidate will demonstrate compassion, empathy and excellent communication skills when working with individuals experiencing distress or complex mental health needs. They will actively participate in clinical and case management supervision, quality improvement activities and ongoing professional development, maintaining high standards of clinical practice and professional conduct at all times.
