Brief Description of Job Role and Department
The post holder reports to the Team Manager or Assistant Team Manager within the Adult First Contact Service. Professional supervision and clinical governance are provided by a Lead Occupational Therapist to ensure safe, effective assessment practice.
The post holder serves as a specialist Trusted Assessor, focusing on streamlining the delivery of equipment and adaptations (e.g., stairlifts, ramps, accessible bath/shower). By managing a caseload of straightforward adaptation cases, the post holder reduces system delays. The post holder acts as a bridge between the service user and the adaptation delivery process, identifying risks, assessing home environments, and recommending practical solutions that promote independence.
The post holder will work in multidisciplinary teams, maintaining up-to-date knowledge of legislation, regulations, and adaptation guidance. They will contribute to service improvement by identifying bottlenecks in the assessment process and suggesting, implementing, and reviewing new ways of working to enhance efficiency and service user outcomes.
Representative Accountabilities
Delivery of straightforward adaptation assessments and management of a caseload, ensuring timely, efficient, and high-quality service outcomes.
Identification and implementation of process improvements within the adaptation pathway to reduce waiting times and enhance the service user experience.
Liaison with service users, carers, and external agencies (e.g., equipment stores, contractors) to ensure seamless adaptation delivery.
Maintenance of continuous professional development, ensuring all assessments comply with current safety standards and legislative requirements.
Budgetary Accountabilities
Efficient, effective, and economic use of council resources. Demonstrate value for money by prioritising cost-effective, readily available solutions before recommending major adaptations.
Manage equipment and adaptation costs in line with local policy, ensuring correct procurement and efficient use of the Integrated Procurement Equipment Hub.
Specific Accountabilities
Undertake Trusted Assessments for straightforward home adaptations (e.g., stairlifts, ramps) within established legislative and safety frameworks.
Screen referrals to determine if a case is suitable for a Trusted Assessor or requires escalation to an Occupational Therapist.
Conduct home environment assessments, identifying risks and recommending practical, person-centered solutions that maximize independence.
Maintain accurate records of all assessments, recommendations, and outcomes, ensuring data quality and GDPR compliance in all systems.
Work collaboratively with multidisciplinary teams, including OTs, Social Workers, technical officers, and housing services, to facilitate integrated, timely adaptation delivery.
Monitor and review the progress of adaptations, acting as the primary point of contact for the service user during the adaptation process.
Identify and report safeguarding concerns, adhering to multi-agency procedures to protect vulnerable individuals.
Contribute to the ongoing development of the Trusted Assessor service, including peer support, mentoring, and the review of process outcomes.
Actively participate in supervision and training, demonstrating a commitment to building and maintaining clinical and technical competence.
Ensure all health and safety standards, including manual handling best practices, are strictly adhered to during site visits and assessments.
