Summary

Pennine Care NHS Foundation Trust is pleased to advertise two permanent, full-time opportunities for qualified Mental Health and Wellbeing Practitionersto join our team working across theBury Neighborhood Mental Health Service.

This role is a key part of the ongoing transformation of community mental health services for adults and older adults, helping to bridge the gap between primary care and specialist mental health services, 

The focus of the role is to provide early assessment, brief psychological interventions, and support individuals to access appropriate community and mental health services. You will help people to identify achievable goals, promote self-management, and connect patients with relevant resources in the local area.  You will provide short-term psychological interventions, including group sessions. You will play an integral role in the multi-disciplinary team,  supporting formulation and sharing expertise on appropriate psychological interventions.

To work alongside clients, families, carers and multi

disciplinary team members to co-ordinate care, supporting

collaborative decision making about care and treatment.

 

To deliver a set of wellbeing-focused psychologically informed

interventions aligned to cognitive – behavioural principles,

based on the best evidence available that address problems

often experienced by people severe mental health problems

who often who have experienced trauma.

 

To deliver specified wellbeing focused psychologically informed interventions in line with best available evidence, under close supervision from a practitioner, psychologist or CBT therapist including:

o  Behavioural activation and graded exposure

oTeaching problem-solving skills

oImproving sleep

 oRecognising and managing emotions

o  Guided self-help for a range of conditions, including anxiety and low mood

o  Building confidence
