Summary

Primary responsibilities The following are the core responsibilities of the Advanced nurse Practitioner. There may be, on occasion, a requirement to carry out other tasks including routine nursing. This will be dependent upon factors such as workload and staffing levels: a. Independently assess, diagnose, treat and manage patients presenting with acute and chronic illnesses. b. Undertake comprehensive history taking, physical examinations and clinical assessments. c. Request, interpret and act upon appropriate investigations. d. Implement and evaluate evidence-based treatment plans. e. Manage urgent presentations, ensuring timely escalation where necessary. f. Undertake home visits where appropriate. g. Maintain accurate and contemporaneous clinical records ensuring that snomed codes are used effectively h. Support the delivery of diabetes care within the practice including conducting annual diabetic reviews in accordance with national guidelines. Optimise glycaemic control through medication review and lifestyle interventions alongside management of overall cardiovascular risk. i. Initiate and adjust diabetes medications, including insulin where appropriately trained and within scope of practice. j. Monitor and manage complications associated with diabetes in line with scope of practice. k. Support patients with education on self-management, diet, exercise and medication adherence. l. Work proactively to improve diabetes outcomes and support QOF performance and enhanced service delivery. m. Collaborate with the wider multidisciplinary team, including community diabetes services and secondary care specialists. n. Manage and review patients with long term conditions including Diabetes, Hypertension, Asthma, COPD, Cardiovascular disease, Chronic Kidney Disease, Heart failure - implementing and evaluating individual treatment plans for chronic disease patients. o. Provide holistic care aimed at prevention, early intervention and optimisation of treatment including promoting healthy lifestyles and preventative healthcare. p. Deliver personalised care planning and encourage self-management and shared decision making. q. Support national screening and vaccination programmes. r. Undertake medication reviews. s. Monitor effectiveness and adverse effects of treatments in line with scope of practice. t. Promote safe and cost-effective prescribing in line with scope of practice. u. Participate in clinical audit and quality improvement initiatives. v. Support implementation of national and local clinical guidance. w. Participate in significant event analysis and clinical governance activities. x. Communicate effectively with patients, relatives and carers. y. Work collaboratively with GPs, nurses, pharmacists, healthcare assistants and administrative staff. z. Liaise and refer patients appropriately to secondary care, community services and other health and social care partners where required. aa. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health. bb. To provide routine nursing care to patients as required in accordance with clinical based evidence. cc. To process pathology results as required and where within scope of practice and competencies. dd. Support members of the HCA/nursing team, providing guidance, leadership and mentorship when necessary. ee. Participate in local initiatives to enhance service delivery and patient care. ff. Support and participate in shared learning within the practice. gg. Continually review clinical practices, responding to national policies and initiatives where appropriate.
