Summary

Longcroft Clinic is seeking an enthusiastic, experienced and patient-focused Practice Nurse to join our friendly multidisciplinary team on a fixed-term maternity cover contract. The successful candidate will work as an autonomous practitioner within their professional scope while collaborating closely with GPs, healthcare professionals and administrative staff to deliver high-quality, evidence-based nursing care to our registered patient population. The Practice Nurse will provide a comprehensive range of clinical services including chronic disease management, cervical screening, immunisations, wound care, health promotion, NHS Health Checks and management of minor illness, whilst supporting the practice in delivering safe, effective and responsive primary care. Main Duties and ResponsibilitiesClinical Responsibilities Deliver high-quality nursing care in accordance with the NMC Code, national guidance and practice policies. Undertake comprehensive patient assessments and provide appropriate treatment, advice and follow-up. Deliver cervical cytology in line with the NHS Cervical Screening Programme. Administer childhood, adult and travel immunisations and seasonal vaccination programmes including influenza and COVID-19. Undertake wound assessment, dressings, suture and clip removal, compression bandaging and leg ulcer management. Perform venepuncture, ECGs, blood pressure monitoring, urinalysis, spirometry (where trained) and other diagnostic procedures. Provide NHS Health Checks and cardiovascular risk assessments. Assess, triage and manage patients presenting with minor illness within agreed clinical protocols. Recognise deteriorating patients and escalate concerns appropriately. Long-Term Condition Management Undertake reviews for patients with long-term conditions including: Diabetes Asthma COPD Hypertension Cardiovascular disease Monitor treatment plans and ensure evidence-based management. Support achievement of QOF and other quality improvement targets. Refer patients to GPs or other healthcare professionals where appropriate. Health Promotion Promote healthy lifestyles and disease prevention. Provide smoking cessation, weight management, alcohol reduction and exercise advice. Educate patients to improve self-management of long-term conditions. Support national screening and vaccination programmes. Patient Care Deliver compassionate, patient-centred care. Maintain patient dignity, privacy and confidentiality. Advocate for patients and involve them in decisions about their care. Identify safeguarding concerns and follow practice safeguarding procedures. Care Coordination Arrange investigations including blood tests, urine samples and swabs. Interpret results within competence and escalate where necessary. Refer patients to community, secondary care and allied health professionals. Liaise effectively with multidisciplinary teams to ensure continuity of care. Infection Prevention and Clinical Governance Maintain high standards of infection prevention and control. Ensure compliance with cold chain and vaccine storage requirements. Participate in clinical audits, significant event reviews and quality improvement projects. Report incidents, near misses and clinical risks in accordance with practice policy. Information Management Maintain accurate, timely and comprehensive clinical records using EMIS Web. Ensure all consultations are appropriately coded. Maintain confidentiality in accordance with GDPR and NHS Information Governance standards. Professional Responsibilities Maintain NMC registration and professional revalidation requirements. Participate in appraisal, mandatory training and continuing professional development. Keep up to date with current clinical guidelines and best practice. Work within professional competence and seek support where appropriate. Contribute to the development of nursing protocols and clinical pathways. Team Working Work collaboratively within the multidisciplinary practice team. Support colleagues and contribute to a positive working environment. Participate in practice meetings, education sessions and service development. Supervise or support healthcare assistants and student nurses where appropriate. General Responsibilities The post holder will: Act professionally at all times. Undertake any other duties appropriate to the role as requested by the Partners or Practice Manager. Person Specification The successful candidate will demonstrate: Essential Registered Nurse (NMC). Experience working within General Practice or Primary Care. Chronic disease management experience. Cervical cytology qualification. Immunisation and vaccination competencies. Venepuncture and wound management skills. Excellent communication and interpersonal skills. Good IT skills and experience using clinical systems. Ability to work independently and as part of a multidisciplinary team. Commitment to continuous professional development. Desirable Experience using EMIS Web. Spirometry competency. Independent prescribing qualification. Mentorship or teaching experience. Experience supporting Quality Improvement and QOF achievement.
