Summary
Health Care Assistants are equipped with the knowledge, understanding, skills, attitudes and behaviours relevant to this role. Working under supervision of the teams clinical leads and the wider multi professional team, you will play an integral role in planning, organising and delivery of clinical care in line with service and patient needs. Work as part of the Enhanced Care Home Team to provide and monitor care, under direct or indirect supervision. Develop relationships across Primary, secondary, and social Care services to support integration of the role across health and social care economy. Act as a link between physical health and mental health to provide a holistic approach for patients. After undertaking additional training, you will support in the promotion of vaccinations and routine immunisations, venepuncture, and other relevant clinical tasks as required by the practice, and in line with your any enhanced training, development and competence Supporting personalised care planning and support plans. Assisting with the collection and collation of data on needs related to health and well-being. Inputting data onto patients' records. Improve safety and quality of care at every opportunity. Contribute to the delivery of integrated care. Maintain accurate records in line with The Dudley Foundation Trust policies and appropriate patient electronic record. Enhance own performance through continuous professional development. Attending team meetings, supervision sessions and training courses. Work in accordance with The Dudley Foundation Trust policies, including infection control, safeguarding, information governance, equality and diversity, health and safety, equal opportunities. Ensure confidentially is always maintained. Participate in practice audits, quality improvement activities and service. Recognise and work within limits of own competence, recognising when necessary to refer to others for assessment. Be always professional and responsible for own actions. Effectively manage own time, workload, and resources.
