Summary
1 Clinical Leadership & Governance Provide professional leadership to practice nurses, HCAs, phlebotomists, and other clinical support staff. Ensure compliance with CQC standards, infection prevention and control (IPC), safeguarding, and clinical governance requirements. Lead on development, implementation, and review of clinical policies, protocols, and standard operating procedures. Oversee clinical audits, significant event reviews, and quality improvement initiatives. Act as the clinical lead for chronic disease management pathways (e.g., diabetes, respiratory, cardiovascular) Lead on national campaigns, relevant incentive schemes and initiatives practice(s) have committed to. Lead the nursing team in the planning and implementation of local guidelines, protocols and standards, and of local projects or initiatives. Work as an effective and responsible team leader, supporting others and exploring the mechanisms to develop new ways of working Lead nursing team activities that create opportunities to improve patient care Communicate nursing view to Management Team. Delegate appropriately, adopting the principles of safe practice and assessment of competence of nurses. Promote the role of the nursing team in the provision of care Actively contribute to practice/organisation level meetings, delivering the nursing agenda ensuring cohesion within the nursing team, communication within it and outside of it Support and encourage development in line with nursing career pathways, include development of advanced roles with a strong focus on workforce planning, recruitment, retention, supervision and appraisal Lead others to make realistic self-assessment of their knowledge and skills identifying training needs, co-ordinating CPD, ensuring staff maintain competencies and mandatory training. Also, challenging any complacency and actions that are not in the interest of the public and/or users of service Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model to achieve the highest standards of quality for the organisation 2 Clinical Practice Deliver highquality, evidencebased nursing care ssess, plan, develop, implement, provide and evaluate treatment programmes Prioritise health problems, intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Manage and treat patients with Chronic Diseases in line with professional competence, working to national and local policies and practice needs. Diagnose and treat minor illnesses. Review medication for therapeutic effectiveness, in accordance with evidence based practice and national/practice protocols, and work with patients to support adherence to prescribed treatments Support and manage health needs of women presenting for family planning and cervical cytology consultations Implement and oversee vaccination and immunisation programmes for both adults and children including patients travelling abroad Make professionally autonomous decisions and be accountable for safe, evidence based, cost effective, individualised patient care Make appropriate referral to other members of the primary, community and secondary team. Signpost appropriately Where the post holder is an independent prescriber ensure safe, effective and appropriate medications are prescribed as defined by current legislative framework and local and practice guidelines. Work on the ground clinically with other nursing professionals to actively lead shoulder to shoulder on the ground Promote a learning environment with a no blame culture Lead the QOF strategy and delivery for LTC management, maximising income Responsible for managing and maximising ICB, PHE and NHSE driven targets (Childhood Immunisations, adult vaccinations, Flu campaigns, cervical cytology programme, contraception & sexual health services, travel health advice & vaccinations) Longterm condition reviews (e.g., asthma, COPD, diabetes, hypertension) Professional leadership and expert clinical advice on all aspects of wound care Support delivery of PCN services such as enhanced access, anticipatory care, and personalised care initiatives Provide clinical advice to the wider team and act as a point of escalation for complex cases 3 Service Development & Performance Strive to ensure the nursing service is delivering a consistently high quality service promoting a positive, patient centred culture in all corners of the organisation Contribute to strategic planning and service redesign within across practices and PCNs Lead initiatives to improve QOF, IIF, and enhanced service performance Monitor clinical KPIs, patient outcomes, and operational efficiency Implement evidencebased practice and support innovation in care delivery Interpret national strategies and policies into local implementation strategies that are aligned to the values and culture of general practice Deliver care as an individual and team according to NSF, NICE guidelines and evidence-based care, assessing effectiveness of care delivery through peer review, benchmarking and formal evaluation and leading on the maintenance of quality governance systems for the nursing team Lead along with Managing Director and the Board, participating in shared learning at team, practice and wider organisation and assess the impact of policy implementation on care delivery Prioritise, organise and manage own and others workload in a manner that maintains and promotes quality whilst balancing the priorities of the practice/area where the nursing service is provided Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, including non medical prescribing quality, implementing improvements where required Evaluate patients response to health care provision and the effectiveness of care Work within policies regarding family violence, vulnerable children and adults, substance abuse and addictive behaviour, and refer as appropriate Recognise and work within own competence and professional code of conduct as regulated by the NMC Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedure
