Summary
We are recruiting a skilled and compassionate Support Worker to deliver a proactive, outreach style package of support to an adult with complex needs, in their own home and community. This is not a traditional task based home care role. The focus is on relationship building, gaining trust and promoting engagement, so that the individual feels supported to maintain their independence and wellbeing on their own terms.
You will need patience, resilience and genuine skill in engaging someone who may find it hard to accept support, alongside sound practical judgement in coordinating appointments, correspondence and day to day problem solving.
About The Person You Will Support
The person you will support lives with depression and associated mental health needs, and may present with low motivation, avoidant behaviours and self neglect at times. Building a therapeutic, trusting relationship is central to the role, as engagement cannot be assumed and must be earned over time. They also live with HIV, and support should be delivered with informed, non judgemental understanding of this alongside their broader health and wellbeing needs.
The Support Required
• A proactive, relationship led, outreach style approach rather than a task and time home care model
• Patient, persistent engagement work to build trust with someone who may initially avoid or decline support
• Support with maintaining self care and daily wellbeing, delivered sensitively and at the person's own pace
• Coordinating and supporting attendance at appointments, and managing correspondence and paperwork
• General problem solving and practical support to help the person manage day to day life
• Careful observation to identify and promptly escalate any signs of deterioration or relapse
• Working collaboratively with the wider care and clinical team supporting the person
