Summary

Community Wellbeing Connector (mental health) Do you want to play a key role in improving mental health and wellbeing within local communities? We have a great opportunity to join Northpoint as a Community Wellbeing Connector (Mental Health), working as part of a partnership to transform community mental health support across Leeds within our Community Mental Health Services (CMHS) service. You will join us on a full time, permanent basis (35 hours per week) with a salary of £32,597 per annum (NJC Scale 6, SCP 20). As a Community Wellbeing Connector, you will play a key role in connecting people aged 18+ with local opportunities, activities, services and community resources, while providing person-centred support to individuals experiencing complex mental health needs across Leeds. You will be responsible for: Providing person-centred support to people to achieve improved wellbeing, recovery and greater independence Working alongside individuals to understand “what matters to me”, developing personalised support plans that reflect their strengths, interests, priorities and aspirations Building strong relationships with local organisations and voluntary community sector partners, including primary care to connect people with activities, services and opportunities that improve their quality of life and to provide joined up support Please note, this is an office-based role at The Small Mill, Leeds, due to the nature of the work and the need for regular in-person support. What are we looking for in our ideal Community Wellbeing Connector? We are looking for someone who is organised, reliable and enjoys working in a varied, people-facing role. A relevant Level 3 qualification in Health and Social Care, Mental Health or a related subject (or equivalent experience in mental health support) Experience of supporting people with mental health needs or complex lives within statutory, voluntary or community settings Knowledge of voluntary, community and statutory services within Leeds and an understanding of the wider factors affecting health and wellbeing, and experience of partnership working, building effective relationships and working collaboratively across organisations Experience of managing a varied workload or caseload and responding appropriately to changing priorities An understanding of safeguarding, confidentiality and risk management If you have the skills, knowledge and experience we are looking for to be our Community Wellbeing Connector and are passionate about making a difference, please click ‘Apply’.
