Summary

The Referral Support Care Navigator will form an integral part of the multidisciplinary team approach incaring for the physical, social and psychological needs of patients going through faster diagnosis referrals,vague systems and screening pathways where there is a suspected cancer.

Work as a Health/Social Care Assistant to assist with the coordination and management of patientsundergoing diagnostic, screening fast track and treatment at The Royal Wolverhampton NHS Trustto support patients both on and off treatment.

Provide information to patients related to first appointments, treatment/follow-up and signpost toappropriate healthcare providers within remit of role and guidance.

Support the fast-track team to book appointments where service dictates.

Provide telephone advice and triage where appropriate, working within defined protocols andguidelines. Document and communicate outcomes as per service specification.

Macmillan Cancer Support and Information Support Manager in developing and facilitating Healthand Well Being Programmes. This will involve contributing and delivering education to patient andcarers, including information around health promotion, disease prevention and supportingself-management of their rehabilitation or long-term condition. This could be across hospital sitesand off site locations
