Summary

Main Duties and ResponsibilitiesClinical Responsibilities Deliver high-quality nursing care within own scope of practice. Undertake chronic disease management clinics, including but not limited to: Diabetes Asthma COPD Hypertension Coronary heart disease Administer vaccinations and immunisations in line with national and local programmes. Conduct cervical cytology screening in accordance with current guidance. Perform wound care management, including dressings and removal of sutures/clips. Provide travel health advice and vaccinations as appropriate. Carry out health promotion and lifestyle interventions to support patients in achieving positive health outcomes. Undertake NHS Health Checks and routine health assessments. Perform venepuncture and other clinical procedures as required. Support infection prevention and control measures within the practice. Recognise and respond appropriately to safeguarding concerns. Maintain accurate, contemporaneous patient records using the clinical system. Patient Care Develop and maintain effective relationships with patients and carers. Provide evidence-based advice, education, and support to patients regarding their health conditions. Promote self-management and preventative healthcare initiatives. Ensure patients are treated with dignity, respect, and compassion at all times. Team Working Work collaboratively with GPs, nurses, healthcare assistants, pharmacists, and administrative staff. Participate in regular practice and clinical meetings. Contribute to service development and quality improvement initiatives. Support colleagues and foster a positive team environment. Professional Responsibilities Maintain registration with the Nursing and Midwifery Council (NMC). Work within the NMC Code of Professional Conduct and professional competencies. Participate in annual appraisal and continuing professional development. Keep clinical knowledge and skills up to date through relevant training and education. Comply with all practice policies and procedures, including confidentiality, information governance, health and safety, and equality and diversity.
