Summary
Practice Nurse – Stockport
We are currently seeking an enthusiastic and experienced Practice Nurse to join a well-established and patient-focused GP practice in Stockport. This is an excellent opportunity to become part of a supportive multidisciplinary team dedicated to delivering high-quality primary care services to the local community.
The successful candidate will be passionate about providing excellent patient care, promoting health and wellbeing, and supporting the management of long-term conditions.
Main Duties and Responsibilities
• Deliver evidence-based nursing care within a primary care setting.
• Undertake chronic disease management clinics, including:
• Diabetes
• Asthma
• COPD
• Hypertension
• Cardiovascular disease
• Perform cervical cytology (smears).
• Administer childhood and adult immunisations and vaccinations.
• Conduct NHS Health Checks and health promotion activities.
• Provide wound care, dressings, and removal of sutures.
• Carry out venepuncture, ECGs, and other routine clinical procedures.
• Deliver travel health advice and vaccinations where appropriate.
• Maintain accurate patient records using clinical systems.
• Participate in clinical audits, infection prevention initiatives, and quality improvement projects.
• Work collaboratively with GPs, Healthcare Assistants, Pharmacists, and the wider Primary Care Network team.
Essential Criteria
• Registered Nurse with current NMC registration.
• Experience working within General Practice, Primary Care, Community Nursing, or a similar setting.
• Competent in chronic disease management.
• Experience undertaking immunisations and vaccinations.
• Excellent communication and patient engagement skills.
• Ability to work autonomously and as part of a multidisciplinary team.
Desirable Criteria
• Cervical Cytology qualification.
• Independent Prescribing qualification.
• Experience using EMIS Web or SystmOne.
• Long-Term Condition management qualifications.
• Travel Health certification.
