Summary
Work as part of a multidisciplinary team within West Northumberland Primary Care Network. Assess, diagnose and manage patients presenting with a range of acute and long-term health conditions. Provide same-day clinical assessment and urgent care support. Undertake home visits and develop appropriate management and treatment plans. Support the delivery of person-centred care and promote patient self-management. Carry out clinical assessments, investigations and health reviews within scope of practice. Refer patients appropriately to primary, community, secondary care and voluntary sector services. Support the delivery of anticipatory care planning and preventative healthcare initiatives. Maintain accurate, timely and contemporaneous clinical records. Work collaboratively with GPs, nurses, pharmacists and wider healthcare professionals. Contribute to multidisciplinary team meetings and service development activities. Ensure care is delivered in line with local and national policies, guidelines and best practice. Participate in clinical supervision, training and continuing professional development. Promote a safe, effective and compassionate approach to patient care. Support quality improvement activities and the ongoing development of PCN services.
