Summary

ProfessionalThe post holder will: Work as an autonomous practitioner within the standards of professional practice outlined by the Chartered Society of Physiotherapists (CSP) and Health and Care Professions Council (HCPC). Be professionally and legally accountable for all aspects of own work. Maintain close liaison with other senior colleagues in the team and ensure effective communication with all team members for day-to-day running of the service. Keep updated of current issues with the profession, taking responsibility for personal growth and development such as reading relevant journal and research papers, participating in training opportunities and support research / quality improvement projects. Maintain and record ongoing continuing professional development, using a professional portfolio. Attend professional body / clinical interest group meetings as appropriate. Participate in the delivery of training for physiotherapy students and fulfil clinical educator role. Contribute to multidisciplinary and uni-disciplinary in-service training programmes. Actively contribute to and be supported in developing knowledge and skills outlined in Personal Development Plans, sharing learning with colleagues. ClinicalThe post holder will: Undertake specialist assessments of clients with a wide range of physical, cognitive and communicative impairments arising from neurological conditions with multiple co-morbidities and complex needs, including end of life conditions. Provide comprehensive assessment, treatment and management to a caseload of adult patients with a wide range of neurological conditions following local operational policies and agreed standards and guidelines. Analyse and provide specialist interventions based on clinical reasoning and evidence based practice to address therapeutic goals and optimise independence. Monitor the effectiveness of interventions through goal setting, outcome measurement and evaluation of practice. Be responsible for prioritising, delegating and managing own caseload and workload, including achieving agreed activity targets. Work collaboratively with individuals, clients, families and carers, members of the multidisciplinary team and external agencies to ensure a coordinated approach to service delivery. Discuss, advise and plan appropriate goals with clients, carers and the multidisciplinary team. Effectively communicate with all appropriate people and organisations to provide and receive all relevant information pertaining to each client. This includes clients with complex emotional, physical, cognitive and communication impairments. Use a self-management approach to empower individuals to take responsibility for their rehabilitation. Respect clients dignity, privacy, culture and wishes during treatment, with a commitment to equitable access to rehabilitation. Maintain and develop close links with colleagues in the acute sector, local authority, voluntary organisations and other members of primary healthcare services. Be accountable for own professional practice and the support staff and students you are responsible for. To work autonomously, ensuring personal responsibility for following lone working policies and procedures when accessing the community and clients place of residence. Attend multidisciplinary team meetings, case conferences, family meetings and other professional meetings as required. Participate in the Trust staff appraisal scheme and supervision structure. Be responsible for adhering to infection prevention and control policies within community settings. Be responsible for safekeeping, cleanliness and maintenance of equipment, ensuring standards of infection prevention and control are met, including reporting on RADAR all incidents, accidents and complaints to the supervisor and/or manager. Participate in ongoing review of service delivery and any developments in the service. Information ManagementThe post holder will: Keep accurate and contemporaneous records of assessment and treatment in accordance with standards set and input data on to the electronic records system (Epic). Provide accurate and timely statistical data, i.e. SSNAP. Education and Professional DevelopmentThe post holder will: Contribute to the planning, organisation and delivery of training for staff in the service and from other disciplines and organisations through in-service training, supervision, joint working and practical demonstrations. Contribute to student clinical education and development. Assist with service development and service promotion. Undertake all mandatory and statutory training in line with Trust policy. Maintain and record ongoing professional development through formal peer review and use of a professional portfolio.
