A Vacancy at Guy's and St Thomas' NHS Foundation Trust.

An excellent opportunity has arisen for physiotherapists with an interest in neurorehabilitation and community working to join the Lambeth and Southwark Community Stroke and Neuro Rehabilitation Service (CSNRS).

The service is recruiting to x3 Specialist Physiotherapy posts. We will be offering x2 posts which are permanent 12-monthly rotations between the Community Stroke Team and the Community Neuro Rehab Team. The 3rd post is a 12-month fixed-term contract with the Community Neuro Rehab Team.

Our service aims to provide client-centred, evidence based multidisciplinary community neurorehabilitation to adults in the London Boroughs of Lambeth and Southwark.

There are five teams within the CSNRS including:

• Community Stroke Team

• Community Neuro Rehab Team

• 2b Neurorehabilitation Beds (Pulross Centre)

• Southeast London Neuro Navigation Service

• Community Spasticity Service

You will be expected to provide a high standard of physiotherapy, undertaking all aspects of physiotherapist duties as an autonomous practitioner.

Regular supervision and support is provided and there is a robust programme of in-service training to assist your development as a Specialist Physiotherapist.

This is a rotational Band 6 Specialist Physiotherapist post within the Community Stroke and Neuro Rehabilitation Service. The post holder will rotate between the Community Stroke Team and the Community Neuro Rehabilitation Team. We are also offering a 12-month fixed term contract in the Community Neuro Rehab Team.

The post holder will provide specialist client-focussed Physiotherapy to individuals as part of the multidisciplinary team and will work closely with other members of the team to ensure a coordinated approach to client care.

The post holder will independently and effectively manage a caseload of individuals with a neurological diagnosis.

The post holder will provide specialist Physiotherapy assessment and treatment for adults in their own homes and other community settings.

They will use evidence based, client-centred principles and a self-management approach to:

• Assess, plan, implement and evaluate interventions.

• Encourage patients to participate actively in their rehabilitation.

• Maximise independence and support quality of life.

The post holder will be expected to achieve agreed individual activity targets (face-to-face and/or virtual contacts) and be flexible in supporting wider caseload management.

The post holder will be expected to supervise junior staff, incl. Clinical Assistants and Physiotherapy Students as required.

They will participate in regular supervision, annual appraisal and continuing professional development.

Professional

The post holder will:

• Work as an autonomous practitioner within the standards of professional practice outlined by the Chartered Society of Physiotherapists (CSP) and Health and Care Professions Council (HCPC). Be professionally and legally accountable for all aspects of own work.

• Maintain close liaison with other senior colleagues in the team and ensure effective communication with all team members for day-to-day running of the service.

• Keep updated of current issues with the profession, taking responsibility for personal growth and development such as reading relevant journal and research papers, participating in training opportunities and support research / quality improvement projects.

• Maintain and record ongoing continuing professional development, using a professional portfolio.

• Attend professional body / clinical interest group meetings as appropriate.

• Participate in the delivery of training for physiotherapy students and fulfil clinical educator role.

• Contribute to multidisciplinary and uni-disciplinary in-service training programmes.

• Actively contribute to and be supported in developing knowledge and skills outlined in Personal Development Plans, sharing learning with colleagues.

Clinical

The post holder will:

• Undertake specialist assessments of clients with a wide range of physical, cognitive and communicative impairments arising from neurological conditions with multiple co-morbidities and complex needs, including end of life conditions.

• Provide comprehensive assessment, treatment and management to a caseload of adult patients with a wide range of neurological conditions following local operational policies and agreed standards and guidelines.

• Analyse and provide specialist interventions based on clinical reasoning and evidence based practice to address therapeutic goals and optimise independence.

• Monitor the effectiveness of interventions through goal setting, outcome measurement and evaluation of practice.

• Be responsible for prioritising, delegating and managing own caseload and workload, including achieving agreed activity targets.

• Work collaboratively with individuals, clients, families and carers, members of the multidisciplinary team and external agencies to ensure a coordinated approach to service delivery.

• Discuss, advise and plan appropriate goals with clients, carers and the multidisciplinary team.

• Effectively communicate with all appropriate people and organisations to provide and receive all relevant information pertaining to each client. This includes clients with complex emotional, physical, cognitive and communication impairments.

• Use a self-management approach to empower individuals to take responsibility for their rehabilitation.

• Respect clients’ dignity, privacy, culture and wishes during treatment, with a commitment to equitable access to rehabilitation.

• Maintain and develop close links with colleagues in the acute sector, local authority, voluntary organisations and other members of primary healthcare services.

• Be accountable for own professional practice and the support staff and students you are responsible for.

• To work autonomously, ensuring personal responsibility for following lone working policies and procedures when accessing the community and client’s place of residence.

• Attend multidisciplinary team meetings, case conferences, family meetings and other professional meetings as required.

• Participate in the Trust staff appraisal scheme and supervision structure.

• Be responsible for adhering to infection prevention and control policies within community settings.

• Be responsible for safekeeping, cleanliness and maintenance of equipment, ensuring standards of infection prevention and control are met, including reporting on RADAR all incidents, accidents and complaints to the supervisor and/or manager.

• Participate in ongoing review of service delivery and any developments in the service.

Information Management

The post holder will:

• Keep accurate and contemporaneous records of assessment and treatment in accordance with standards set and input data on to the electronic records system (Epic).

• Provide accurate and timely statistical data, i.e. SSNAP.

Education and Professional Development

The post holder will:

• Contribute to the planning, organisation and delivery of training for staff in the service and from other disciplines and organisations through in-service training, supervision, joint working and practical demonstrations.

• Contribute to student clinical education and development.

• Assist with service development and service promotion.

• Undertake all mandatory and statutory training in line with Trust policy.

• Maintain and record ongoing professional development through formal peer review and use of a professional portfolio.
