Summary
To establish and maintain effective links with internal/external agencies. To take proactive steps to ensure service accessibility. This includes joint assertive outreach with Julian House and leading engagement groups across the county. To offer practical support to service users, joint working alongside the lead professional. Sharing personal lived experience, which may include revisiting distressing life events Managing professional boundaries while drawing on personal experiences Navigating multi-agency systems and overcoming barriers to service access Organise and prioritise workload in an environment with interruptions and changing prioritise.
