Working as part of the SouthWest Thames Renal & Transplantation Unit team at St Helier Hospital, you will liaise with clinical, nursing and administrative staff, providing administrativesupport, ensuring that patients are efficiently assessed for dialysis transfer to satellite dialysis sites . You will be central to the team ensuring all renal dialysis patients are booked for clinic appointments, pre-transfer appointments, investigative treatments, diagnostic procedures and will track patient progress throughout You will also ensure that patients are informedwith reasonable notice, and that all relevant paper work is completed for their transfer.This includes ensuring that all parts of the patient pathway are promptly and accurately recorded on the appropriate PAS system(s) including Cerner iClip, Renal EPR (CV5).
· To provide administrative support to consultant/clinical teams including the management of clinician’s diaries/timetables. Ensure consultants’ timetables, outpatient clinics and operating/treatment sessions are fully and appropriatelyutilised. Attend regular meetings with consultant teams, MDT teams, clinical staff, clinical directorates /departments and/or general managers as required for efficientprocesses.
· To ensure that the information on patient letters, is recorded accurately and comprehensively on the Trust PAS system(s). Check patient identity details including all demographic information during patient encounters.Amend letters and patient leaflets to ensure that information distributed to patients is accurate and current.
· To maintain efficient systems and processesrelatingtopatientattendancesandtransfers.Thisincludes maintaining accurate computerised and manual records.
· Facilitate the timely production and dissemination of all patient and healthcare professional correspondence, including clinical correspondence, capacity tracker spreadsheets, and production of patient MDT meeting minutes to clinical teams, to achieve quality measures.
· Ensure that all patient transfer and capacity information isaccuratelyrecordedonall appropriatedatabases,including, but not limited to, CV5, Cerner iClip, PACS etc.
