Summary

Work alongside Public Health and One Bromley colleagues to build and strengthen partnerships, including through collaboration with voluntary and community sector partners, local NHS providers, Primary Care Networks (PCNs) and Community Champions. Work alongside Public Health programme leads and the senior team to lead on projects linked to commissioned programmes and Public Health priorities. Support, co-ordinate and where required lead commissioning, mobilisation and contract monitoring arrangements for Public Health contracts. Manage local steering/working groups, working with the Chair to set agendas, prepare reports and co-ordinate the delivery of actions plans. Responsible for the identification of good practice in the management of lived experience; engaging with residents/service users, and making recommendations to the Public Health Action Board in developing new practices. Provide expertise in identifying gaps in the current provision of programmes relating to Health Inequalities and plan appropriate innovative improvements if needed. Instrumental in developing strong working relationships across the whole Health and Wellbeing System to support vulnerable residents and deliver tailored programmes to reduce health inequalities and strengthen pathways of care. Facilitate multi agency meetings and co-ordinate delivery of action plans for specific projects. Collate and analyse information from a range of partners and write up the findings in standardised quarterly and annual reports. Share complex data in an accessible way with partners using established protocols and information sharing agreements. Prepare presentations and present reports and data to a wide range of key stakeholders across the borough, One Bromley organisations and South East London Integrated Care System. Remain informed of new legislation, central government policies, local authority, health and other partners policies and practices relevant to Public Health. Maintain a thorough knowledge of the key priorities set out by the strategic boards and partnerships that this role will service. Undertake any other duties commensurate with the level of the post, as required to ensure the efficient and effective running of Public Health.
