Summary
An exciting opportunity has arisen within the Central Cheshire Integrated Care Partnership Adult Physiotherapy Service foraPhysiotherapistlooking to specialize withinMusculoskeletalservices.
We are looking for afull timepermanent Band 5 Physiotherapist to join ourMusculoskeletalrotations which rotate on a 6 monthly basis across our South and Vale Royal Clinics. These include a variety of Primary Care settings and also offers the opportunity to gain essential acute experience at Leighton HospitalOutpatients Department .The Primary care sites areSandbach,Alsager,Middlewich,Winsfordand Nantwich where you will work as part of the Musculoskeletal service incommunity clinics and GP practices.
This position focuses on Musculoskeletal and Orthopaedic rehabilitation of GP and Consultant referrals. We can offer you the opportunity to work with a range of experienced senior physiotherapists who will support your Continuous Professional Development. There is the opportunity to attend in-service training and peer review sessions.
You will have current HCPC registration, a full UK driving licence and access to avehicle, orbe able todemonstratemeans of meeting the travel requirements of the post. • To contribute to theday to day running of the MSK department, showing responsibility andaccountabilityfor the operational delivery of the service.
• To autonomously,competentlyand effectively manage a MSK caseload. Demonstrating a good awareness of scope of practice and when to seek help.
• To respect the individuality, values, cultural and religious diversity of patients, contributing to the provision of a service sensitive to these needs.
• To utilise effective communication skills to ensure clinical advice and treatment programmes are delivered sensitively, taking into consideration the needs of carers.
• Torecognise indications and contraindications to treatment modalities, effectivelygaining full informedconsent for treatment. Demonstrating understanding of legal implications of gaining informed consent and when to seek help or escalate.
• Understand and act to minimisebarriersto treatmentsuch as cognitive impairment, hearing, visual or speech impairment.Demonstrating the ability to effectively communicate and document.

• To undertake clinical assessment,consideringall relevant information and clinical reasoning to formulate evidence based individualised treatment plans in agreement with the patient.
• To implement individual and/or group interventions for patients.
• Tomonitorprogress and review treatment plans and interventions tofacilitatethe achievement ofpatientcentredgoalswithina timelymanner.
• To provideappropriateverbaland written information for patients and carers where relevant.
• To develop andmaintaingood working relationships,demonstratinga high levelof professionalism.
• Work effectivelywith other healthcare professions, socialcareand otherappropriate agencies, to ensure integrated andhigh-qualitypatient outcomes.
• Tomaintainaccurate, completeclinical documentationwithina timelymanner,in line with professional standards, local policies and procedures.In the event ofmitigating circumstances, it is theindividual'sresponsibility to highlight this to the relevant MSK Physiotherapy Manager.
• To follow CCICP peer review policy when considering referral onwards to other agencies or referral back to target. Adhering to emergency care pathways withappropriate communicationand documentation.
• Toparticipatein CCICP MSK audits asrequired.
• To ensure the safe andtimelydischarge of patients, liaising with other agencies asrequired.
• Ensure EMIS caseloads are effectively managed, dischargingin a timely manner. Participating in EMIS mentorship to ensure effective management of patients care.
• Tobe responsible forsafe and competent use of equipment including the provision of support, education and training to other staff, patients and carers asrequired.
• To contribute to departmental clinical audit
• Participates in the development,maintenanceand delivery of best practice guidelines, led by special interest groups and enhanced practitioners.
• Toprovidementorship, teaching and education tostudentsandassistants withinCCICP MSK Physiotherapy services.
