Summary

Brief Aim: -

To build trusting therapeutic relationships and to provide Recovery focused and person-centered support to enable people with mental health needs to live as independently as possible in the community.

Responsible for:

1. Main tasks:

• To develop warm and trusting relationships with service users, encouraging them to express their needs, views and concerns.

• In line with R.I.S.E Recovery Ethos, to work with service users to develop and implement person centered, outcome based (Recovery) support plans that outline goals and future aspirations and achieve positive outcomes.

• To help create an empowering environment that promotes the service user’s self-esteem and enables them to express choice and preference and to make decisions about their lives.

• To support service users in the maintenance of their emotional wellbeing and mental health. To promote recovery by supporting service users manage their symptoms through the use and contribution to Recovery plans and other tools such as Wellness Recovery Action planning.

• To support service users to manage their own finances liaising with the Department of Work and Pensions, Job Centre Plus and local support agencies to maximise their income and to maintain a good quality of life.

• To support service users to manage their own accommodation and to maintain the safety, hygiene and comfort of their homes.

• To support service users to participate in their local communities to engage with sources of formal and informal support and to enjoy the rights and responsibilities of citizenship.

• To support service users to build and sustain social networks, including family and friends and informal and formal peer support.

• To support service users to develop and express their personal identity and to meet their spiritual and cultural needs.

• Through person centered planning to encourage service users to identify their strengths and interests and to further their recovery through the identification of education and work opportunities as well as social and leisure activities.

• To work within agreed risk management guidelines and to assist service users to reduce risk to themselves and others. To promptly report all concerns regarding risk to senior staff and relevant agencies

• To maintain and develop Service User files as appropriate.

• To adhere to all written Policies, procedures and good practice agreed by R.I.S.E.

• To participate in the weekly shift Rota system, including overnights and weekends.

• To positively promote the service, taking part and contributing to Service User CPA’s, G.P appointments and raising the profile of the service.
