Summary

Activity Development and Implementation

In liaison with parents and member - to plan and develop the delivery of services

provided by ‘Integrating+’ in line with the needs of the people registered with the organisation.

Management and Supervision

To manage service delivery activities through ensuring, members have appropriate

and up to date risk assessments and a constant monitoring of each members

Person-Centred-Plan.

Work with Service Users

To provide support and supervision for members with a disability to enable them to

participate in agreed activities as safely and independently as possible. To assist in

developing our members life skills and reaching realistic objectives set.

Session location

We use a centre for planning and a small range of activities; however, the majority of the successful candidate's working day will be spent supporting members within the local community. This includes accessing a wide range of community facilities, activities, and services that have been chosen by the member as part of their person-centred support plan, promoting independence, inclusion, and meaningful participation.
