Summary
Lincolnshire County Council has an exciting opportunity within our West Adult Community Wellbeing Team for an Advanced Practitioner – West Review Team.

About the Role
Working alongside the Team Manager, you will play a key role in supporting high-quality, strengths-based and person-centred practice, helping to embed innovative ways of working and driving continuous improvement across the service.
As an experienced Social Worker, Occupational Therapist, or Nurse, you will work with autonomy, managing some of the most complex cases referred to Adult Care and Community Wellbeing. You will also coach, mentor, and support colleagues, contributing to the development of a skilled, reflective, and confident workforce.

As an Advanced Practitioner within Community Wellbeing, you will be responsible for providing expert professional leadership while maintaining a varied and complex caseload.
You will be a role model for excellent practice, providing advice, guidance, and support to colleagues, and helping to ensure adults receive the right support at the right time.
This is a pivotal role where you will combine advanced practice, leadership, safeguarding expertise, and partnership working to influence outcomes for individuals and the wider service.
You must have the ability to travel across Lincolnshire and occasionally beyond county boundaries to meet the needs of the service.
You will:
Manage a varied and complex caseload, undertaking assessments, support planning, reviews, and risk management.
Lead and support colleagues in delivering strengths-based, personalised, and outcome-focused practice.
Provide mentoring, coaching, and reflective supervision to team members.
Support less experienced practitioners through co-working, guidance, and case consultation.
Take a lead role in complex safeguarding cases, ensuring legislation, policies, and procedures are appropriately applied.
Promote person-centred approaches that maximise independence, wellbeing, and community connections.
Contribute to service development, quality assurance activities, and continuous improvement initiatives.
Act as a Practice Educator, mentor Newly Qualified Social Workers, and support student placements.
Deliver workshops, group supervision, and learning opportunities to develop practice across Adult Care.
Build strong working relationships with health partners, voluntary organisations, communities, and other agencies to deliver integrated support.
Deputise for the Team Manager where required.
You will have the opportunity to shape practice across the team while supporting adults to achieve the outcomes that matter most to them.
