The care coordinator will attend meetings for planning improvement on performance targets e.g. LTCs, screening, immunisation, NHS health checks, LD checks and a range of other service related targets. 10. The care coordinator will work closely with the practice manager and clinical service leads to support the setting up of clinics. They will help plan forward capacity and monitor effective appointment utilisation by patients. This will include the administration of clinical rotas and appointment sessions on the clinical system, allowing for annual leave, study leave and other absence. 11. The care coordinator will act as 'session lead' for mass vaccination clinics, coordinating the rota, supporting internal and external clinicians to start up IT systems and troubleshoot e.g. passwords. Coordinate vaccine allocation for the session and to individual clinicians. Minimise vaccine wastage and ensure that the cold chain is maintained at all times. Top up clinical supplies and ensure a tidy clinic. Safely ensure that clinical waste e.g. sharps is properly disposed. Monitor clinic throughput and escalate effectively to manage queues. Respond to patient queries on eligibility and book appointments. Support volunteers e.g. queue marshals with patient queries and feedback about the service. Report vaccine usage / wastage and update fridge stock sheets.
