There is an exciting opportunity available within the in- patient Palliative care team at the Royal Free site. We are a dynamic and supportive team who offer various learning opportunities whilst in post. The CNS contributes to the provision of specialist palliative care in the acute hospital setting for patients facing life-limiting illnesses (within the remit of the multi- disciplinary team) accepting patient referrals to agreed criteria and managing their own caseload with the support of the Lead Nurse and Consultant Nurse. They will provide advice, education and support to patients, their carers and other health and social care professionals. They will contribute to the development and smooth running of the service. The post holder will work to agreed professional, departmental and Trust boundaries as required to provide patient centred seamless care and will take part in the 7 day / week service to hospital patients and North Camden community patients. Roles and responsibilities · Maintaining Cardiology Medical rota in accurate and timely manner to ensure all planned (annual, study leave) and sickness absences are reflected in real time. · Effectively coordinating Cardiology Medical rota ensuring reallocation of staff to cover shortfalls and any gaps are escalated in a timely manner. · Ensuring timely distribution of most recent rota to all stakeholders · Facilitating and attending regular meetings with clinical rota managers · Continuously liaising with Assistant service manager to stay up to date with upcoming changes in establishments, identifying the impact on the rota and assisting in taking mitigation steps Administration office · Take overall responsibility for efficient running of the administrative function, maintaining effective office/ departmental systems and processes. · Work with other members of the wider team to ensure quality standards are adhered to. · Ensure that the team is aware of relevant Trust policies and procedures relating to HR processes and service delivery. · Develop new processes for the service with a view continuously improving the patient experience. · · Take responsibility for patient/client care in terms of dealing with complex patient queries, particularly where patients are distressed and emotional. · Manage complex patient queries referred from other staff to an appropriate resolution.
