The post holder will identify complex and specialist needs and will develop, implement and evaluate programmes of care. Working as a practitioner with a high degree of autonomy, the post-holder will manage undifferentiated and undiagnosed problems using advanced clinical skills by incorporating physical assessment and diagnosis and be supported to do this by senior members of the MDT. The post holder will also identify patients suitable for virtual ward monitoring and be proficient in fitting the appropriate technology following training and support from the team and will be responsible in assessing unwell patients at home on request of the virtual ward hub.
